Flipping plays in the huddle is realistic, as many teams mirror the playbook.
Flipping plays at the line, as Madden does, is unrealistic and would likely cause motion penalties in a real game.
I like motion, I like audibles, but I like realism, and flipping plays at the line is not that.
Players in Madden often flip plays as a way to defeat the AI. It is a cheat of sorts. Real world defenses would adjust.
